This is a 9-week contract position offering full-time hours (40 hours per week) on day shifts. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for providing quality care to elderly and long-term care residents.\n\nKey Responsibilities : \n

• Patient Care : Provide high-quality care to residents, including administering medications, monitoring vital signs, and assisting with activities of daily living.\n
• Collaboration : Work closely with Registered Nurses (RNs), physicians, and other healthcare team members to develop and implement individualized care plans.\n
• Documentation : Maintain accurate and timely documentation of patient care, medications, and treatment plans in accordance with facility policies.\n
• Patient Advocacy : Serve as an advocate for patients' health and well-being, ensuring their comfort and safety throughout their stay.\n
• Compliance : Follow all regulatory guidelines and facility policies related to infection control, safety, and confidentiality.\n
• Communication : Effectively communicate with patients, families, and staff regarding care needs, treatment changes, and progress.\n
• Emergency Response : Respond appropriately to medical emergencies, providing immediate care and support until further assistance is available.\n\nQualifications : \n
• Current LPN license in the state of New Hampshire / Compact.\n
• Experience in long-term care or geriatrics preferred.\n
• Ability to work effectively in a team environment and independently as needed.\n
• Strong communication and interpersonal skills.\n
• Ability to adapt in a fast-paced and changing environment.\n
• CPR and First Aid certification required (or willing to obtain).\n
