Senior CommUnity Care of Colorado PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.

Pay : $53,700-$67,300 annually

Schedule : Monday - Friday, 8 : 00am - 5 : 00pm, occasional weekends.

  • The Therapeutic Recreation Specialist is responsible for the recreational assessment of participants, development of a plan of care designed to meet the participant’s needs and coordination with other staff, oversight of care planned interventions, management of day center

    programming and task oversight of activities aides.

    QUALIFICATIONS :

  • EDUCATION : Degree in Therapeutic Recreation or similar field. May consider equivalent experience in lieu of degree.
  • EXPERIENCE : Must have one year’s experience working with the frail or elderly population. Three (3) years’ experience providing therapeutic programming to frail or elderly population preferred.
  • Must have medical clearance for communicable diseases and up-to-date immunizations after having direct participant contact.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Skills and Knowledge :

  • Knowledge of the principles’ practices, standards and techniques of the provision of therapeutic and / or recreational activities.
  • Knowledge of how to use and maintain materials and equipment needed to provide recreational activities.
  • Knowledge of safety requirements when providing and implementing recreational activities.
  • Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff and the public.
  • Ability to observe participant behaviors and report observations to appropriate staff.
  • Able to identify problems and recommend solutions.
  • Ability to work both independently and in team settings.
  • Ability to communicate clearly and effectively.
  • Proficient in the use of computers.
  • Ability to follow healthcare standards of documentation in the electronic medical record.
  • Willingness to work at either PACE Center routinely and as needed.
  • ESSENTIAL FUNCTIONS :

  • Functions as an engaged member of the SCCCO Interdisciplinary Team (IDT). Maintains regular attendance at and participates in IDT meetings; communicates participant changes and collaborates on plan of care decisions with the IDT. As requested by the Care Manager, attends and participates in
  • Care Team meetings.

  • Conducts an in-person initial comprehensive recreational assessment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant enrolled in SCCCO.
  • As indicated by the Care Team, conducts in-person re-assessments as needed; reassessments may be required semi-annually, when there is a change of condition / status, or when triggered by the Care Team or IDT.
  • Manages and develops supportive social, therapeutic, spiritual, cognitive, physical, and recreational programing in the SCC Day Centers that meet the interests, needs and capabilities of participants. Ensures incorporation of care planned interventions into the Day Center programming.
  • Follows the Therapeutic Recreation philosophy of programming to the highest ability level and assisting, through adaptive programming and services, each participant to reach their individual potential.
  • Provides skilled one-to-one therapeutic recreation interventions as indicated by the plan of care.
  • Advises and supports incorporation of therapeutic recreation interventions into homecare services.
  • Occasionally or as needed, leads and / or assists in large groups, small groups, and individual therapeutic interventions or activities (exercise, arts and crafts, games, life review, sensory, cognitive, spiritual, etc.).
  • Provides recreational education and training to support the Activities Assistants and Day Center staff.
  • Maintains participants’ medical record and fulfills Senior CommUnity Care charting and reporting requirements. Adheres to all SCCCO documentation policies.
  • Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development, and recommended supplies for in-home use.
  • Provides oversight and coordinates implementation of outings and outdoor recreation events / programming.
  • Provides oversight and coordinates opportunities for community partners to take part in providing additional services in the day centers, IE : education, entertainment, etc.
  • Encourages participant involvement in recreation opportunities and provides necessary support to participants, ensuring their needs are met.
  • Partners with Outreach Department to further develop and promote the PACE Day Center and community relationships.
  • Works cooperatively with other staff and departments to meet Senior CommUnity Care participants’ needs.
  • Assists participants with non-skilled Activities of Daily Living (ADL) as needed.
  • Provides ongoing education / communication to participants in one-to-one settings, group settings, and participant councils of program changes, events, and addresses concerns as needed.
  • Provides task oversight of Activity Aides to ensure successful facilitation of day center programming and individual care plan needs.
  • Provides ongoing education and support to Activity Aides to ensure fulfillment of quality, intentional programming is being provided on a daily basis.
  • Responsible for ensuring yearly competencies are completed for Activity Aides and supportive staff, as needed.
  • Maintains and consistently demonstrates high customer service.
  • Is responsible for adherence to program cultural standards including supporting the development and integration of these standards within the organization through modeling, coaching, and accountability.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Acts within the scope of his or her authority to practice.
  • Follows all Senior CommUnity Care program policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
  • Maintains safe working environment. Follows Senior CommUnity Care Program Safety policies and procedures.
  • Participates in continuing education classes and any required staff and training meetings.
  • Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
  • Participates in and supports Quality Improvement Initiatives.
  • Performs other related duties as requested or required.
