• Full time 32 Hours a week RN Case Managers (flexible schedule)\n\nJob Requirements : \n
• Comprehensive knowledge and skill in current nursing practice.\n
• Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.\n
• Excellent knowledge of the OASIS process.\n
• Knowledge of the community resources.\n
• Ability to make meaningful observations, evaluations and reports.\n
• Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.\n
• Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.\n
• Valid driver's license.\n\n
