About the Role :
As a Care Coordinator, you’ll be the heartbeat of daily operations—ensuring clients receive outstanding care and caregivers feel supported. This role requires someone highly organized, proactive, and team-oriented.
Responsibilities :
Coordinate and manage daily scheduling of services
Resolve client and caregiver concerns promptly
Answer calls with warmth and efficiency
Maintain accurate care plans and records
Collaborate with the care team for seamless service
Participate in on-call rotation as needed
Requirements :
3+ years of home care industry experience
Experience with AxisCare or similar scheduling software preferred
Strong multitasking, organizational, and problem-solving skills
Excellent communication and teamwork abilities
Comfortable with technology and quick to learn new systems
Self-starter who goes above and beyond
