This is a great opportunity for nurses with previous LTAC or ICU experience to make an impact in a high-acuity setting. Responsibilities : Deliver high-quality patient care and ensure the nursing care plan is followed. Perform nursing assessments within 8 hours of admission. Collaborate with interdisciplinary team to identify care goals and develop individualized plans. Provide patient updates for care team meetings and participate in weekly re-evaluations. Document assessments, interventions, and outcomes in accordance with policy. Support rehabilitation efforts and patient / family education to promote optimal outcomes. Initiate and maintain discharge planning from admission. Communicate effectively across shifts and roles to ensure patient safety and continuity of care. Supervise LPNs, CNAs, and orientees. Participate in team building, equipment management, and unit improvement efforts. Follow all safety protocols including Safe Patient Handling and Infection Control. Assume charge nurse responsibilities as requested after orientation.
