DISTINGUISHING FEATURES OF WORK :
Directly relates to the CILA Site Manager for administration and program issuesWorks with the QIDP for clinical issuesIs responsible for the consumers while they are in the CILA home to see that they are safe and following their treatment planAlso responsible for overseeing the CILA homesILLUSTRATIVE DESCRIPTION OF DUTIES :
Provide direct service care as requiredAssist consumers with implementing their Individual Treatment Plan in their homes, community and other environmentsHelp consumers with crisis situations involving supervisors and clinical staff as needed / requiredAssist consumers with the administration and training of medications taking direct orders from certified medical personnelMake sure that the home is a safe and clean environment, i.e. – assisting in emergency drills, checking safety equipmentAssist consumers in purchasing and preparing food and other items as needed for them in their homeTransport consumers in agency vehicles to appointments and activities in the community as outlined in their Treatment PlanHelp consumers to participate in the design of the array of community support services tailored to their needsMaintain sensitivity to the service population’s cultural and socioeconomic characteristics.Be responsible for staying updated on changes in consumers’ treatment plans, medications and overall careADMINISTRATIVE DUTIES :
Follow policies and procedures of the CILA Site and Human Support ServicesFollow and implement all medication procedures as assignedParticipate in the planning of consumers’ Individual Treatment PlanDo related progress notes per the goals assigned to you for billing purposes and provide all the services indicated in the treatment plan and all other documentation necessary in a timely mannerProvide necessary information regarding consumer care to site manager and / or appropriate staff in a timely mannerRequirements :
TRAINING AND JOB REQUIREMENTS :
Meet with CILA Site Manger monthly at scheduled site meetings to assist with consumer monitoring and planningAttend full CSN trainings quarterly to stay updated on consumer care and / or changes affecting CSN and Human Support ServicesSuccessfully complete all required certifications of CSN, HSS and the State of Illinois that relate to your position as scheduledAttend trainings outside required monthly trainings that relate to your position as mutually agreed uponPHYSICAL REQUIREMENTS :
Must be 18 years of ageMust have valid driver’s licenseHave adequate transportation to be able to move from site to siteAbility to drive a regular size and larger size van or willing to be trainedDrive in large citiesPresent High School diploma or GEDAble to lift 50 pounds and walk flights of stairsHave normal or corrective eyewear and hearing devicesHave clarity of speech to be understoodGood hearingConstitution to handle body excrementsMust be emotionally stable enough to react appropriately to a consumer in crisisOTHER :
