Job Details :

  • Advise patients and community members concerning diet, activity, hygiene and disease prevention
  • Analyze records, reports, test results or examination information to diagnose medical conditions.
  • Collect, record and maintain patient information, such as medical history, reports, and examination results.
  • Explain procedures and discuss test results or prescribed treatments with the patient.
  • Immunize patients to protect them from preventable diseases
  • Manage and treat common health problems
  • Help us to enhance our existing New York State "Patient Center Medical Home" Standard
