Job Description :

Seeking a dedicated Occupational Therapist for a home health contract opportunity in Missoula, MT. This role involves evaluating patients, creating care plans, and delivering direct rehabilitation services in patients’ homes. Ideal for an experienced OT passionate about providing personalized, community-based care.

Key Responsibilities :

Conduct evaluations and establish care plans for patients in a home setting

Provide skilled therapy services per physician orders and care plans

Document evaluations, treatment progress, and any adverse events

Coordinate with multidisciplinary teams and attend care conferences

Perform supervisory visits for OTAs and CNAs as required

Participate in Quality Assurance and Performance Improvement initiatives

Maintain accurate records for visits, mileage, and schedules

Plan efficient patient care routes and maintain daily communication with scheduling team

Maintain licensure and ethical practice in accordance with state board standards

Requirements :

BS or higher in Occupational Therapy from an accredited program

Active Montana OT License – Required

BLS Certification – Required

Minimum 2 years of OT experience – Required

Valid driver’s license and personal auto insurance

Home health experience and OASIS familiarity preferred
