Oklahoma Licensed Care Manager

A company is looking for a Care Manager (RN).

Key Responsibilities

Develops and facilitates complex care management activities for members, focusing on physical and mental health needs

Coordinates care between members, families, and providers to ensure adequate person-centered services

Monitors member progress and updates care plans as necessary to meet individual goals and needs

Required Qualifications

Active Registered Nurse (RN) license required

Must reside in Oklahoma

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in healthcare case management or managed care

Experience with care planning and resource coordination preferred
