Need flexibility in hours? We got you covered! Gateway Rehab Center has an immediate need for a Clinical Case Manager located at our Westmoreland, PA center. You will work directly with our residential patients. The Case Manager will be responsible for completing assessments and treatment plans, facilitate group sessions, and help facilitate movement to the next level of care.

Responsibilities

• Provides case management support to the inpatient clinical team.
• Completes all documentation in patients’ charts regarding case management and treatment-related activities.
• Facilitates group sessions when needed.
• Networks with referral sources and outside treatment providers and other agencies.
• Coordinates and communicates with other Gateway / Greenbriar programs and departments.
• Provides oversight of patient daily activities as needed, including unit supervision.
• Provides direct patient care as needed.
