Annual Salary : $350,000.00 Base Salary Productivity bonus of $5 / RVU which would equal out to about 20-30k per year. Sign on bonus. Continuing Medical Education (CME) Allowance : $3,000.00 - per year Benefits Details : benefit start on day 1 except for the 401k Health Insurance (Medical + Dental + Vision) EmployeePays : 20% / Company Pays : 80% Long Term & Short-Term Disability ( EmployeePays : 25%
