Responsibilities
The Nurse Practitioner is responsible for delivering comprehensive risk assessments in patients' homes or via telehealth, operating within their scope of practice as assigned by the Collaborative Physician.
Key Responsibilities
Conduct detailed in-home health risk assessments to identify active and chronic diseases, as well as physical, mental, and social needs.Gather patient histories, perform examinations, and establish diagnoses.Document patient visits according to NCQA standards.Measure and record vital signs as needed.Coordinate case management referrals and collaborate with the patient's primary care provider.Maintain clear communication with patients, caregivers, agency nurses, and other healthcare providers for accurate diagnoses.Adhere to OSHA Universal Precautions while performing clinical duties.Ensure patient confidentiality is upheld at all times.Attend required meetings and in-services, contributing to committees as needed.Engage in professional development activities and uphold professional licenses and affiliations.Team Collaboration

Required Knowledge, Skills, and Experience
Active and unrestricted medical license.Current CPR Certification.Board eligible or board certified in family medicine.Strong EHR skills.Preferred Knowledge, Skills, and Experience
Experience or training in geriatric care.Aptitude for teamwork and fostering effective working relationships with patients and medical staff.Conditions of this Role
Ability to adapt to varying weather conditions and different patient home environments.Full range of motion for handling and lifting patients, including manual and finger dexterity, coordination, normal vision, and hearing abilities.Regularly lift and carry items weighing up to 50 pounds.Possible travel in an automobile or van for up to 150 miles daily in urban and rural settings, with the ability to drive if necessary.Pay Transparency
