Job description

$7,500 Sign-On / Retention Bonus for Qualified Candidates

AS A LICENSED PRACTICAL NURSE, you will be administer skilled nursing care to patients on an intermittent basis in the patient's place of residence in close coordination with other disciplines and caregivers involved in the patient's care. This is performed in accordance with physician orders and plan of care and in accordance with agency policies and procedures.

Fleet vehicles available for eligible employees
2026 Subaru Crosstrek
Fleet Vehicles are permitted to be used for work AND personal use
All fuel costs for a fleet vehicle (for work AND personal) are covered by PHH

QUALIFICATIONS :

High school diploma or equivalent required

Current LPN licensure in the state of Pennsylvania required

Previous Home Health / Hospice experience preferred

CPR certification from the American Heart Association within the first year of hire required
