Description :

We are seeking a Case Manager to oversee a daily caseload of patients in inpatient units with a variety of care needs.

The role includes attending interdisciplinary rounds.

You will complete initial assessments and facilitate the development of care plans in collaboration with patients, families, caregivers, and the healthcare team.

Consistent communication with medical teams and patients regarding progress and post-discharge care needs is required.

You will interact with post-acute care referral sources to establish a plan.

Responsibilities also include documenting and placing orders for home health, durable medical equipment, and nutritional needs within the electronic medical record system.
