About the Role
In this role, you will conduct comprehensive health assessments during visits that can last up to one hour, diving deep into members’ health histories, medication adherence, social environments, and other pertinent risks. This approach will provide invaluable insights that regular office visits may overlook.
- Perform adult and geriatric assessments, including medical history, diagnosis, treatment, health education, and necessary referrals.
- Order and administer point of care and lab-based tests as needed.
- Collaborate with a range of professionals, including physicians, case managers, social workers, and family members, to ensure optimal care.
- Engage in the education of members and collaborate with their Primary Care Physicians as needed.
- Provide care directly in the member’s residence, whether at home or in skilled nursing facilities.

Master's Degree or relevant experience and completion of NP or PA licensure.Current NP or PA licensure in your practice state with prescriptive authority or the ability to acquire it.Board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care specialties.Current certifications in BLS, ACLS, or CPR.Preferred one year of experience as a Nurse Practitioner or Physician Assistant – we welcome new graduates to apply!Valid driver's license and reliable automobile insurance.Proficient computer skills and familiarity with medical record software.Strong verbal and written communication abilities.The Matrix Culture
