Registered Nurse (RN) Care Manager
Role Summary
Seeking a compassionate and experienced Registered Nurse Care Manager to join their
dedicated care team. This dynamic role involves assessing patient needs, delivering skilled nursing care,
and developing individualized care plans to promote health and quality of life for an aging population. The
RN Care Manager will collaborate closely with a multidisciplinary care team to coordinate and deliver care
across clinical, home, and community settings.
Duties
- Deliver high-quality, person-centered nursing care across clinic, home, and contracted facility
settings.
Conduct timely nursing assessments and implement nursing interventions based on individualizedcare plans.
Monitor acute and chronic conditions, initiate appropriate interventions, and ensure continuity ofcare.
Coordinate follow-up care, including specialist visits, labs, and treatment plans.Provide health education to patients, families, and caregivers on disease management, medicationadherence, infection control, and self-care.
Support care planning by contributing insights and recommendations in team meetings.Delegate tasks to clinical and homecare aides, ensuring they meet competency standards.Participate in on-call triage and respond to urgent patient needs as required.Engage in quality improvement initiatives, utilization reviews, and interdisciplinary communication.Ensure documentation is timely, accurate, and compliant with all policies and regulations.Foster positive relationships with patients, families, colleagues, and community partners.Uphold safety and infection control standards while maintaining patient confidentiality.Other duties as assigned
Required Qualifications
Current and valid RN license in the state of practiceMinimum of 1 year of RN experienceBLS certification (or obtained within 90 days of hire)Reliable transportation, valid driver's license, and minimum required liability auto insuranceClearance of background check, including immunization and communicable disease requirementsDesired Qualifications
Bachelor of Science in Nursing (BSN) preferredExperience with older adult or medically complex populations preferredBackground in home care, long-term care, or managed care environmentsStrong clinical assessment, care coordination, and patient education skillsFamiliarity with electronic health records and healthcare softwareTeam-oriented mindset with excellent communication and problem-solving abilitiesPassion for delivering exceptional care to vulnerable populations in diverse settingsCompany Information

Location & Work Type
