Position Summary :
The Certified Nurse Assistant (CNA) will provide essential care to patients as outlined in a specific care plan developed by an RN Case Manager. You will assist patients in meeting their basic needs and activities of daily living under the supervision of an RN. This role is certified by the State of Connecticut, and you may have the opportunity to provide hospice care as part of your responsibilities.
Education and Certification Requirements :
High school diploma or GED required.Certification as a Home Health Aide or CNA with the Connecticut State Nurse Aide Registry.Completion or enrollment in a Basic Approved Training Program as defined by Licensure Regulations.Experience :
Previous experience as a CNA / HCA preferred.Skills and Competencies :
Ability to read, write, and communicate effectively in English.Possession of a current driver's license and access to a vehicle.Ability to demonstrate a sympathetic attitude and work harmoniously with patients and their families.Commitment to personal growth through required in-service education.Excellent communication skills with a commitment to maintaining confidentiality.Essential Duties and Responsibilities :
Assist patients with personal care and activities of daily living (ADLs), including bathing, grooming, toileting, feeding, and mobility support.Provide household services related to patient care, including meal preparation, laundry, and light housekeeping.Communicate changes in patient conditions to the RN Case Manager or appropriate personnel.Respect and uphold the privacy and dignity of patients at all times.Participate in team activities and adhere to Agency policies and procedures.Your dedicated care makes a difference in the lives of our patients.
