FLSA Classification : Full-Time, Exempt, $31.42- $45.75
Essential Functions :
- Provide therapeutic services to patients and their families.
- Work in collaboration with multidisciplinary team members including external partners.
- Completes required documentation and paperwork including assessments, treatment planning and implementation in timely manner.
- Develop and maintain social services, draft supporting policies, procedures and guidelines for the program. Develop and maintain a knowledge of referral resources throughout the service area.
- Act as a resource person for other members of the healthcare team, provide in-service education and consultation of social and emotional factors related to health problems.
- Maintain knowledge of the field including Medicare, Medicaid and other third-party reimbursement sources.
Required Skills / Abilities :
Working knowledge of clinic policies and proceduresKnowledge and experience working with co-occurring populations, chronic health, mental health, or substance abuse.Strong clerical / administrative skills including computer skills.Ability to make routine decisions and determine proper action based on experience with the limits of policy / procedures.Demonstrated oral / written skills in EnglishExpectations :
