Are you a compassionate and skilled Registered Nurse looking to make a meaningful difference in the lives of patients and families? Cornerstone VNA is seeking a Hospice Admission Nurse (RN) to join our caring and dedicated Hospice & Palliative Care team.

What You'll Do

As a Hospice Admission Nurse, you will :

Perform hospice admissions, assessments, and evaluations.

Develop individualized plans of care with patients, families, and the hospice team.

Educate and support patients and caregivers on end-of-life care needs.

Coordinate services, medications, and durable medical equipment (DME).

Actively participate in interdisciplinary team meetings.

Provide compassionate, patient-centered care while ensuring dignity and comfort.

What We're Looking For

Licensure : Current RN license in NH or compact state (ME preferred as well).

Experience :   1+ year hospice experience preferred.

Strong assessment, decision-making, and teaching skills.

Comfortable with technology and electronic health records.

Independent, self-motivated, and compassionate caregiver.

Valid driver's license and ability to travel in varied weather conditions.

📅 Schedule : Full time, with backup, on-call, and holiday rotation responsibilities. either 4x10 hour shifts or 5x8 hour shifts
