The Patient Navigation Coordinator plays a critical role in ensuring patients receive seamless, personalized support throughout their healthcare journey.
Key Responsibilities :
- Care Coordination : Serve as the primary point of contact for patients navigating complex healthcare processes, including referrals, appointments, and follow-ups.
- Benefits Review : Guide members through understanding their self-funded health plan benefits, including covered services, out-of-pocket costs, and how to access care through our program.
- Eligibility Verification : Review and verify member eligibility for coverage and services, ensuring accurate information is communicated to both members and providers.
- Patient Support : Provide education and resources to patients regarding their diagnoses, treatments, and healthcare options through our program.
- Data Management : Maintain accurate and up-to-date patient records, including tracking progress and outcomes.
- Collaboration : Work closely with other partners such as the Nurse Navigators and client account managers to ensure a seamless member experience.
- Documentation : Accurately document all member interactions and follow up on any pending issues in a timely manner.
- Compliance : Ensure adherence to all HIPAA regulations and company policies regarding patient confidentiality and care standards.
Qualifications :
Minimum of 2 years’ experience in customer service, patient advocacy, care coordination, within the insurance industry or working with self-funded health plans.Familiarity with healthcare systems, insurance processes, and medical terminology.Strong interpersonal and communication skills, with the ability to handle sensitive information empathetically.Skills :
Excellent verbal and written communication skills.Strong problem-solving abilities and attention to detail.Ability to navigate multiple systems and databases.Comfortable using CRM systems and other insurance-related software tools.Ability to empathize with members and provide a high level of support.Ability to work collaboratively in a team-oriented environment.Preferred Qualifications :
Experience working with third-party administrators (TPA) or in a self-funded insurance setting.Bilingual (Spanish) is a plus.Why Join Us?
