Registered Nurse - Admissions Inpatient- $10K Sign on Bonus

$10K Bonus eligible position

POSITION SUMMARY :

The Admission RN is responsible for managing the admission process for patients transitioning from the Emergency Department or other areas to inpatient units. Directly responsible for completing the nursing admission process, including admission assessment, interview, medication reconciliation, and education. Facilitate patient flow into the acute care areas from the Emergency Department or from the Primary Care Physician. Provides the prescribed medical treatment and personal care services to patients in the care of the Organization.

EDUCATION / CERTIFICATION :

  • A current Registered Nurse license from the State of Connecticut is required.
  • Bachelor’s degree required.
  • Certification is highly desirable.
  • Must meet and maintain clinical and managerial competency for areas of assignment including Basic Cardiac Life Support (BCLS) and Advanced Cardiac Life Support (ACLS) during employment in this role.

EXPERIENCE :

  • Two (2) years of RN experience is required.
  • Six (6) months working as an RN within our organization is highly preferred.
  • IV experience is required.
  • COMPETENCIES :

  • Able to communicate effectively in English – both verbally and in writing.
  • Basic computer skills, including Microsoft Office Suite.
  • Interpersonal skills are necessary to effectively communicate with patients, families, and health care team members.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and documentation in patient charts.
  • Demonstrates ability for independent problem-solving.
  • Consistently displays a calm, organized approach to difficult situations by using customer service skills.
  • Serves as a role model in displaying initiative and professionalism with positive problem-solving techniques and willingly performs non-assigned duties as needed.
  • ESSENTIAL DUTIES and RESPONSIBILITIES :

  • Serves as the central point of contact for all ED-to-inpatient admissions during an assigned shift.
  • Coordinates with nursing supervisor, ED charge nurse, inpatient charge nurses, and transport to ensure timely transfer of patients.
  • Initiates the nursing admission assessment and completes the documentation for the nursing admission process. The patient population may include, but is not limited to :

  • The adolescent patient.
  • The young- aged adult patient.
  • The geriatric patient.
  • Coordinates with the staff RN when the nursing admission process is initiated. If the admission nurse is completing another admission, they will communicate to the receiving RN a time frame they will be available to assist with the admission. The receiving RN will be required to initiate the vital signs and physical exam on the admission documentation form prior to the admission nurse arriving.
  • Completes the admission process, including physical assessment, interview, home medication list, Fall and Braden Risk Assessments, Educational Needs Assessment, and Immunization Form, in an accurate and complete manner.
  • Promotes quality improvement initiatives relating to patient throughput and nursing staff satisfaction.
  • Promotes patient satisfaction by completing the nursing admission process in a thorough and timely way using customer service techniques.
  • Ensures patients and families are informed of the admission process and are prepared for the transition to inpatient care.
  • Notifies the charge nurse, manager or nursing supervisor for urgent problems or occurrences.
  • Communicates with appropriate departments or nursing supervisor when a need arises for equipment or supplies.
  • Communicates with the staff nurse caring for the patient by giving a report on the admission assessment findings and patient needs or concerns.
  • If there are no admissions in the hospital, patient discharges will be completed in an accurate and timely manner.
  • Downtime in between completing admissions will be spent completing tasks on the inpatient care units.
  • Rounds to the Emergency Department, SCU, CCS, and Acute Care floors at the beginning of the shift and throughout the shift to inform the charge nurse of their availability and assessing for any admissions on their unit.
  • Carries a hospital-issued communication device (e.g., phone) to enhance communication with the charge nurse, nursing supervisor, and the admission nurse when notifying of admissions.
  • When an Admission Nurse is not required due to census or staffing constraints, the incumbent will be required to take a patient assignment as needed.
  • Performs additional duties as assigned and within the scope of practice.
  • Functions as an IV RN as needed.
  • Participates in performance improvement activities related to throughput and capacity management.
