Location : New York (On-Site at Assisted Living Facilities)

Schedule : Part-Time | No Nights or Weekends

Compensation : $60,000 – $120,000 / year (based on schedule and experience)

1. Position Description

Are you passionate about providing care to the geriatric population? We are seeking a compassionate and skilled Nurse Practitioner to deliver comprehensive primary care services to residents of assisted living facilities. In this role, you will conduct health assessments, manage chronic and acute conditions, develop individualized care plans, and collaborate with interdisciplinary teams. This is a part-time, on-site opportunity with flexible weekday hours—no nights or weekends—ideal for those seeking work-life balance while making a meaningful impact on patient lives.

Job Requirements

2. Requirements

Master’s degree in Nursing and graduation from an accredited Nurse Practitioner program
Unrestricted New York State Nurse Practitioner license
Valid DEA permit to prescribe medications
Eligibility to provide reimbursable services and maintain provider status with Medicare and Medicaid
Minimum of 3 years clinical experience, preferably in geriatrics or long-term care
Experience with eClinicalWorks (eCW) electronic health records is preferred
