What You'll Do :

• Evaluate and treat patients, modifying care plans as needed.
• Perform patient transfers safely and effectively, ensuring the well-being of everyone involved.
• Design personalized care plans that integrate goals, treatment, and discharge plans.
• Educate patients, families, and caregivers using engaging and effective teaching methods.
• Conduct thorough evaluations to determine physical therapy diagnoses and outcomes.
• Document clinical findings in accordance with our Clinical Process Policy.
• Monitor and provide feedback to support personnel, ensuring high-quality care.
• Communicate progress and treatment recommendations with referring providers.
• Complete annual requirements for senior status, including student supervision and in-service training.
