Essential Functions (Key Roles & Responsibilities)

Assess patients upon admission and throughout their stay to monitor progress and adapt care plans accordingly.

Administer medications and treatments accurately, and observe patients for therapeutic and adverse effects.

Monitor, document, and report vital signs, symptoms, and changes in patients' conditions in a timely manner.

Collaborate with an interdisciplinary team—including physical, occupational, and speech therapists—to develop and implement comprehensive rehabilitation care plans.

Provide patient and family education regarding recovery goals, treatment plans, self-care, and discharge preparation.

Modify nursing interventions based on patient responses and functional progress.

Encourage patient participation in therapy and mobility to promote independence and optimize recovery.

Maintain accurate, detailed records in accordance with regulatory and organizational standards.

Ensure compliance with all hospital policies, procedures, and clinical standards.

Perform other related duties as assigned to support unit operations and patient outcomes.
