Home Health Aide I (HHA)
Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse / Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly. Accurately and completely document client care on activity sheet. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements. Valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV) Home Health Aide certification - New York State Education Department (NYSED) Compassionate, patient focused and dependable professional. Critical thinking skills, decisive judgment and the ability to work with minimal supervision. Exceptional documentation and communication skills. Ability to read, write and speak English, ability to follow verbal and written instructions. Medium Work - Exerting 20 to 50 pounds of force occasionally, and / or 10 to 25 pounds of force frequently, and / or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
