• Looking for someone with excellent customer service skills. Strong interpersonal & written communication skills. Ability to multi-task, work independently, & work efficiently in a fast-paced environment with changing priorities.
• Common types of patients & job duties : Dealing with all payers, uninsured, homeless. Completing initial assessments, DC planning (including nursing homes, home health, DME, prior authorizations, etc)Case load / # patients per day / consults per day Caseloads can range from 15-28 on averageEMR : CernerIs this position for UR, discharge planning, or both?Discharge planningWhat areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.) : InpatientExperience or 'Must have' skills : 3 years' nursing experience in an acute hospital setting- Prior care coordination experience in a CommonSpirit / Dignity / CHI hospitalCall Requirement : NoneScrub Color / Dress : Business casual & lab coat Cancellation can cancel up to 4 shifts per 13-week assignment, 3 shifts for assignments that are less than 13 weeks (Hospital may also pay full hours for a particular shift, in lieu of cancellation). Charting BSN License to Trauma Level : Care Shift Required Guaranteed Hours : 40 hours per week Contract Weeks : 91
