Become the Case Manager You Aspire to Be
Collaborate with an interdisciplinary team to tailor treatment plans based on patient needs and preferences.Develop and coordinate comprehensive discharge plans with the team.Engage in planning for a smooth discharge experience for patients.Monitor patient satisfaction regarding quality and service standards.Lead weekly team conferences and coordinate modifications to treatment plans.Complete all necessary documentation and case management addendums.Stay informed about regulations, company policies, and department operations.Analyze case management reports to identify trends and implement improvements.Understand commercial contract levels and insurance requirements.Participate in Acute Care Transfer (ACT) meetings to mitigate transfer trends.Meet with patients and families within 24 hours of admission to establish a strong rapport.Conduct initial assessments of patient goals within 48 hours of admission.Educate patients and families on rehabilitation processes and your role as a Case Manager.Facilitate family conferences as needed to support patient care.Assist patients in obtaining necessary resources to avoid discharge delays.Ensure compliance with regulations related to orthotics and prosthetics.Make timely referrals to post-discharge providers and ensure accurate documentation.Participate in utilization review processes, including data collection and analysis.Engage in on-call case management responsibilities as required.Qualifications
License / Certification :Qualified to conduct independent assessments within your professional scope (RN, SW, OT, PT, ST, Rehabilitation Counseling).Must hold an active license if required for your discipline.Must meet eligibility for CCM® or ACM certification upon entry into this role or within two years of starting.CCM® or ACM certification is mandatory or must be obtained within two years of placement in the Case Manager II position.Minimum Qualifications :
For Nursing, a minimum of an Associate Degree in Nursing is required; RN licensure with a BSN preferred. A diploma is acceptable where allowed by law.For all other licensed or certified healthcare professionals, a minimum of a bachelor's degree is necessary, with a graduate degree preferred.2 years of experience in rehabilitation care is preferred.The Encompass Health Culture
