POSITION SUMMARY

Work with a multidisciplinary team to deliver cardiac rehabilitation services to inpatients, outpatients, telehealth and community customers. Provide exercise training, comprehensive cardiovascular patient assessment, management and education for CV risk factors such as blood pressure, blood lipids, diabetes, tobacco cessation, weight and psychological issues, and counseling for psychological, nutritional and physical activity issues in a safe, efficient, and effective environment. Coordinate and deliver treatment plans to meet the individual needs of each patient.

POSITION QUALIFICATIONS

Minimum Education

Associate degree or equivalent in Nursing

Minimum Experience

Two years in cardiology, critical care, emergency nursing or related field.

Required Registration / License / Certification

BLS upon hire

Registered Nursing license.

Preferred Education

Bachelor’s Degree in Nursing

Preferred Experience

One year of previous cardiac rehabilitation experience or patient education experience.

Preferred Registration / License / Certification

AACVPR Certified Cardiac Rehab Professional (CCRP)
