Position Summary

We are hiring Infectious Disease Nurse Practitioners to provide specialized care across multiple Skilled Nursing Facilities in the Suburbs of Chicago. This part-time role is perfect for experienced clinicians looking for schedule flexibility, autonomy, and the opportunity to make a meaningful impact in long-term care.

Responsibilities

Perform assessments, diagnosis, and infectious disease treatment

Collaborate with physicians and SNF staff on care planning

Develop weekly rounding schedules based on facility needs

Maintain timely, accurate EMR documentation

Qualifications

Active, unrestricted Illinois Nurse Practitioner license

Master’s degree in Nursing (MSN) from an accredited program

Minimum 1 year experience in SNF or long-term care (2+ years preferred)

Excellent interpersonal and clinical decision-making skills

Able to work independently in a fast-paced setting
