We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. We are a physician owned Primary Care Practice serving our community since 1998.

The ideal candidate will provide comprehensive care to patients within the adult and geriatric population, ages 18+. focusing on health promotion and management of acute and chronic illnesses. The NP will work collaboratively with physicians and other healthcare professionals to ensure high-quality patient care in a supportive environment.

Duties would include

• Conduct thorough health assessments, including physical examinations and patient histories.
• Diagnose and manage acute and chronic medical conditions.
• Develop and implement individualized treatment plans based on patient needs.
• Provide education and counseling to patients and their families regarding health maintenance and disease prevention.
• Collaborate with interdisciplinary teams to coordinate patient care.
