Responsibilities :
- Work with the patient and the family to design and implement an individualized plan of care
- Update progress notes and other documentation on an ongoing basis
- Collaborate with other team members, when applicable, in the performance of patient care
Requirements and Qualifications :
- Master's or Doctorate of Physical Therapy from an accredited program
- IL PT licensure
- Passion for pediatric patients
- Desire to partner alongside parents
- Motivated and eager to learn
- Excellent communication skills
- Collaborative, team-building personality style
