As a Homecare LPN, you will leverage your comprehensive education and experience to deliver personalized care in a supportive environment that champions patient autonomy. Your role will include :

• Facilitating communication between patients and the interdisciplinary team.
• Building and nurturing therapeutic relationships while educating patients and encouraging self-advocacy and independence.
• Utilizing modern technology to accurately document the skilled services you provide.
