- What Youll Do
- Conduct Thorough Exams :
- Perform comprehensive physical assessments and closely monitor patient condition changes.
- Prescribe with Confidence
- : Initiate appropriate medications and treatment plans tailored to each patients needs.
- Shape Patient Care
- : Develop and manage care policies while facilitating timely referrals to specialists.
- Document & Communicate Clearly
- : Maintain accurate, timely, and comprehensive records while keeping all team members informed.
- Collaborate to Elevate Outcomes
- : Work closely with physicians and interdisciplinary teams to enhance patient care and results.

- What You Bring
- Licensure :
- Current, unencumbered Nurse Practitioner license
- Masters Degree in Nursing (accredited program)
- Experience :
- 2+ years preferred, with strong background in infectious disease, geriatrics, or adult medicine. (SNF, LTC, palliative, hospice)
- Excellent communication and interpersonal skills with elderly subacute and chronically ill patients
- Knowledge of Medicare, Medicaid, and health plan structures (preferred)
- Ability to travel between facilities 20-25 miles radius
