• What Youll Do
• Conduct Thorough Exams :
• Perform comprehensive physical assessments and closely monitor patient condition changes.
• Prescribe with Confidence
• : Initiate appropriate medications and treatment plans tailored to each patients needs.
• Shape Patient Care
• : Develop and manage care policies while facilitating timely referrals to specialists.
• Document & Communicate Clearly
• : Maintain accurate, timely, and comprehensive records while keeping all team members informed.

• : Work closely with physicians and interdisciplinary teams to enhance patient care and results.

• What You Bring
• Licensure :
• Current, unencumbered Nurse Practitioner license
• Masters Degree in Nursing (accredited program)
• Experience :
• 2+ years preferred, with strong background in infectious disease, geriatrics, or adult medicine. (SNF, LTC, palliative, hospice)
• Excellent communication and interpersonal skills with elderly subacute and chronically ill patients
• Knowledge of Medicare, Medicaid, and health plan structures (preferred)
• Ability to travel between facilities 20-25 miles radius
