Job Description

A Home Care Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self-management and provides support to meet the needs of the patient, family, and community.

Responsibilities

• Complete revisits, SOC / ROCs, recerts, and discharges.
• Manage productivity of 30 patients per week, seeing approximately 6 patients per day.
• Document all patient care in Homecare Home Base EMR.
