Utilizing evidence-based practices, you will guide all aspects of nursing care, ensuring a focus on patient and family-centered care. You will conduct comprehensive assessments, develop individualized care plans, and evaluate patient outcomes effectively. • Conduct thorough nursing assessments, recognizing changes in patient status, and establishing care priorities based on assessments.\n
• Collaboratively develop and update individualized care plans with patients, families, and healthcare team members.\n
• Ensure a seamless transition of care by planning for patient discharge and coordinating necessary referrals.\n
• Deliver direct patient care that aligns with established nursing plans and practices.\n
• Monitor the quality of care and implement improvements based on continuous evaluation.\n
• Actively contribute to a safe care environment by following established protocols and safety standards.\n
• Educate patients and their families by assessing learning needs and delivering tailored teaching plans.\n
• Maintain professional competencies through ongoing education and self-evaluation.\n
• Foster collaborative relationships with healthcare team members to coordinate patient care effectively.\n
• Perform additional responsibilities as assigned to enhance patient care delivery.\n\nSupervisory / Management Responsibility\n
• This position is non-management and will report to a supervisor, manager, or director.\n\nMinimum Requirements\n
• Education : Associate degree in Nursing. Bachelor's degree in Nursing preferred.\n
• Experience : One (1) year of experience as a registered nurse.\n\nIn Lieu Of\n
• Completion of the Prisma Health Nurse Residency Program or a comparable residency from another organization can substitute for one year of experience.\n
• For internal candidates, one year of licensed practical nurse (LPN) experience can be considered.\n
• A nursing diploma with licensure to practice as a registered nurse can also be accepted in lieu of an Associate degree.\n\nRequired Certifications, Registrations, Licenses\n
