Location / Schedule : NYC - 3 locations 60% remote telehealth from home and 2 days per week in clinic

• Scheduled at any of the 3 sites but not required if provider has site preference
• Client is flexible
• Either in clinic 2 days per week, remote from home 3 days per week or in clinic 1 day per week and remote from home 2 days per week

• Mon-Fri (9am-5pm)
• 3 month locum w / option to extend
