Provide compassionate nursing care to patients in home, hospice, or community-based settings.\n
Conduct comprehensive assessments and develop individualized care plans.\n
Deliver wound care, pain management, and other skilled nursing interventions.\n
Collaborate with physicians, social workers, chaplains, and other members of the care team.\n
Educate patients, families, and facility staff on disease processes, symptom management, and end-of-life care.\n
Document all care activities accurately in the electronic medical record.\n
Coordinate care transitions to ensure appropriate levels of treatment and support.\n
Advocate for patients by protecting their privacy and ensuring compliance with regulatory standards.\n\nQualifications\n
Valid driver's license, reliable vehicle, and active auto insurance.\n
Experience in wound care required.\n
Prior hospice, home health, or case management experience strongly preferred.\n
Familiarity with electronic medical records (EMR) systems; Home Care Home Base (HCHB) highly desired, but training available.\n\nPreferred Skills\n
Experience caring for elderly and medically fragile patients in home or facility settings.\n
Strong documentation skills, including narrative assessments and interdisciplinary group (IDG) reporting.\n
Comfort with care planning and coordination across multiple disciplines.\n
