About The Position

Job Title : Registered Nurse

Hours per Shift : 8 Hours (08 : 00 - 17 : 00)

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty.

License : Must have license in hand.

Certifications : CPR, BLS

Must-Have : Proficiency in OASIS and HomeCare HomeBase EMR within the last 6 months.

Description :

Provides coordinated and directed home nursing care according to physician's orders.

Delivers care based on agency policies and procedures through the competent application of the nursing process.

Responsible to the Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Performs home visits and participates in periodic office meetings.

Must have reliable transportation and current driver's license.

Must maintain agency-required liability insurance.

Average number of patients per day is 5-8.

Contractor will be assigned between 5-10 patients per day.

Types of cases include start of care, wound care, and follow-up visits.

Must be willing to travel up to 75 miles from the home clinic.

Mileage is reimbursed at the IRS standard rate.

Traveling from patient home to patient home is compensated.

Commute to and from the first and last patient is not billable.

First-timers are not accepted; recent specialty experience is required.

Weekend requirements vary based on location and profession.

On-call requirements vary per location and profession.

Documentation must be completed within 24 hours of the appointment.

Delayed and incomplete documentation that impacts billing and follow-up patient care will result in a cancellation.

No guaranteed hours with this contract.

Must meet weekly productivity points of 30, equating to 40 hours per week.
