Position Overview :
- As a Home Care Nurse (LPN), you will provide essential nursing care as per the plan established by the RN Case Manager.
- Hospice experience is preferred but not mandatory.
Key Responsibilities :
Deliver skilled nursing services to patients and families, ensuring compliance with state and Medicare regulations.Effectively assess the physical and psychosocial status of patients.Implement care plans in collaboration with the RN Case Manager, adjusting as necessary for optimal patient outcomes.Maintain accurate and timely documentation of all nursing services provided in compliance with regulations and agency policy.Communicate regularly with the RN Case Manager regarding patient care and any concerns.Contribute to maintaining a safe and confidential environment by adhering to HIPAA regulations.Physical and Work Environment :
The role involves direct patient interaction in their homes, community settings, and clinics.Physical demands include lifting, moving, and providing care for patients, which may expose you to communicable diseases.You will encounter stressful situations so the ability to remain calm is essential.Education and Experience :
A graduate of an NLN-approved school of licensed practical nursing.One year of recent medical / surgical nursing experience is preferable; community health nursing experience is a plus.A current license to practice as a licensed practical nurse in Connecticut is mandatory.Why Join Us?
