Position : Registered Nurse (RN) Location : Amarillo, TX Compensation : $40-45 / hour Work Type : Full-Time Department : Clinical Reports To : Director of Nursing (DON) Dotted-Line Collaboration : Executive Director (ED) The Registered Nurse (RN Bridge Nurse) plays a vital role in supporting clinical operations by bridging staffing gaps in unassigned homes and working directly with patients and families to ensure acceptance and utilization of authorized care. This role ensures continuity of care, promotes patient engagement, and supports compliance with clinical and regulatory standards. In addition to clinical responsibilities, the RN Bridge Nurse collaborates closely with the Operations team — including Care Team Directors, Staffing, and the Executive Director — to align field staffing strategies with patient needs. While this position reports directly to the DON, the RN Bridge Nurse maintains a collaborative (dotted-line) relationship with the Executive Director to ensure coordination between clinical and operational priorities. Responsibilities Utilization of Unstaffed HomesProvide direct nursing care in homes without assigned staff to maintain continuity and quality of careComplete assessments and documentation (e.g., T1001) for patients in unstaffed homesIdentify and report trends in staffing gaps and collaborate with the DON, Care Team Directors, and Staffing to address themSupport interim care planning and execution until permanent staffing is secured Patient Engagement & Care Acceptance - Engage patients and families to encourage acceptance of approved care services - Educate on the importance and benefits of home health services - Participate in care plan discussions and family meetings to resolve concerns or hesitations - Collaborate with Patient Coordinators, Care Team Directors, and Operations to align care delivery with patient needs - Location-focused (rather than serving as a member of any individual Care Team) and anchored primarily on staffing unmet skilled need in specific locations Clinical Oversight & Support - Conduct supervisory visits for patients without assigned nurses or those assigned to LPNs - Review and sign Case Management notes to ensure compliance and quality - Support nurse education and mentorship to improve documentation, clinical judgment, and utilization practices - Notify physicians and interdisciplinary teams of changes in patient condition - Serve as a clinical resource for field staff and leadership Operations Collaboration - Partner with the Executive Director, DON, and Care Team Directors to coordinate short- and long-term staffing strategies for clinical coverage gaps - Provide real-time feedback on field staffing challenges and collaborate with and Operations, including Staffing, teams to resolve barriers - Participate in interdisciplinary operations meetings to align clinical staffing with business priorities and patient census changes - Serve as a liaison between clinical leadership and operational leadership to ensure cohesive execution of patient care plans Minimum Qualifications Active Registered Nurse (RN) license in the state of practice (BSN preferred) 3 years of nursing experience; home health or transitional care experience preferred Strong communication and interpersonal skills, with a focus on patient-centered care Knowledge of DOL / EEOICPA guidelines and home health regulations is a plus Proficient in EMR systems; experience with IGEA preferred Compentencies Patient-Centered Communication Adaptability & Problem Solving Clinical Judgment Compliance & Regulatory Awareness Initiative & Follow-Through Cross-Functional Collaboration Work Environment Field-based with travel to patient homes as needed Requires flexibility to respond to urgent staffing needs, conduct supervisory visits, and support nurse education Compensation & Perks
