This role focuses on delivering high-quality patient care in the home setting, managing a varied caseload, and ensuring accurate and timely OASIS documentation. Key Responsibilities :

Conduct patient assessments and complete OASIS documentation

Deliver skilled nursing care based on physician orders and care plans

Monitor and update patient progress

Provide education and resources to patients and families

Communicate with the interdisciplinary team to optimize care outcomes

Qualifications :

Current RN license in MA

2+ years of nursing experience

1+ year of recent home health experience required

Strong proficiency with OASIS documentation and HCHB

Drivers license, dependable transportation, and proof of insurance

Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
