A company is looking for a Senior Wellness Coordinator who will focus on follow-up after hospitalization and assist in meeting HEDIS goals.

Key Responsibilities

Manage requests for discharge planning assistance from facilities

Obtain follow-up after hospitalization details for members in mental health inpatient admissions

Develop relationships with facilities to support discharge planning

Required Qualifications

High School Diploma / GED (or higher)

5+ years of experience in healthcare

2+ years of experience in a managed care environment

Intermediate level of proficiency with MS Office
