Job descriptionHigh school diploma or equivalent. Successful completion of a written home health / hospice competency test and the clinical skills testing. Certification as Home Health Aide (CHHA). Six months of experience with a variety of patients. Valid driver’s license, proof of valid auto insurance, and maintain a driving record in accordance with McLaren’s corporate driving policy and eligibility guidelines. Position Summary :
Under direct supervision, provides personal and homemaking services to ill, convalescing, elderly, and disabled persons in the patients' home.
Essential Functions and Responsibilities :
- Provides personal care of a patient, including bathing, grooming / hygiene, and toileting.
- Takes and records daily vital signs. Reports all vital signs that are outside the normal range.
- Assists patients with positioning / turning, transfers, catheter care, and range of motion exercises.
- Light housekeeping and food preparation.
- Recognizes and reports any unusual, untoward, or new symptoms / changes to the RN.
- May perform functions to support the telehealth program (uninstalls, equipment cleaning and maintenance, installations / setups).
- Performs other related duties as required and directed.
