Schedule :  Full Time

Shift :  Mon – Fri 8 : 00-4 : 30 / On call weekend commitment Monthly / Quarterly

Location :  Jackson, Platte, Clay, Cass Counties in Missouri

The role you’ll contribute :

This position is accountable for providing professional nursing activities as identified and planned by the R.N. Case Manager in the Patient Care Plan to maximize function within the home restoring health or assisting a peaceful death.  The L.P.N. is supervised in the home by the R.N. as often as necessary, but at least every sixty days.
