Summary
Acts as a part of the LifeBridge Care Coordination team. Works closely with patients, and their families, to assist with goal setting and creating and implementing shared care plans.
Works with multiple facility teams to coordinate care and facilitate patient discharge transitions from hospitals, rehabs and other extended care facilities to their Primary Care Provider and team.
Under the general supervision of the Care Coordination Manager and / or the Practice Leadership Team, provides safe, therapeutic and efficient care and services to patients. Supports the care team by functioning as a coach for disease prevention, early screening, testing and treatment, and helps patients manage their chronic diseases.
Assists in the care of patients and instructs patients and their families in appropriate care, as directed by their provider.
Functions as a member of the Care Coordination Team (CCT) to assist in the planning and implementation of patient care based upon sound clinical preparation, knowledge, skills and experience. Care is provided in accordance with the provisions and scope of a Medical Assistant.
Experience : 3-5 years
Education
Preferred : Associate's Degree
Licenses And Certification
Required : Basic Life Support
Preferred : Medical Assistant Certification
