We are seeking a detail-oriented and organized Intake Weekend Scheduling Coordinator to support weekend intake, referral management, and scheduling for our clinical teams.

This position plays a vital role in ensuring smooth communication, timely processing of referrals, and accurate scheduling of weekend home care services. If you thrive in a fast-paced administrative environment and enjoy being a critical support to patient care teams, this role is a perfect fit.

This position may be in-person, hybrid, or remote , depending on agency needs and candidate qualifications.

🔹 What You'll Do

Intake, Coordination & Communication

Collaborate closely with the weekend supervisor.

On weekdays, coordinate VNA services with Intake staff, managers, clinical teams, and referral sources.

Provide updates to Community Liaisons for patient hospitalizations.

On weekends, coordinate home health services and communicate updates to weekday teams.

Referral Management

Receive and process referrals for home care, perinatal, IV therapy, rehab, social services, and support services.

Ensure referral documentation includes required Face-to-Face (F2F) components.

Workflow Processing

Process weekend workflows for intake, billing, and scheduling with accuracy and timeliness.

Document all phone calls and updates in the EMR.

Support continuity of operations by completing limited weekend billing tasks.

Weekend Scheduling Responsibilities

Lead completion of the weekend rehab schedule.

Review intake and weekend supervisor reports to ensure all referrals and visits are accurately scheduled.

Triage scheduling calls and address field staff concerns.

Process urgent workflows such as missed visits and next-day coordination notes.

Call patients on weekends to notify them of next-day admissions.

Assist the weekend RN with additional tasks as needed.

🔹 What You Bring

Prior healthcare experience preferred

Strong organization and prioritization skills

Excellent verbal and written communication

Ability to multitask and work independently

Proficiency with computers, EMR systems, referral portals, and fax-based workflows

Professionalism, adaptability, and problem-solving skills

No licensure required.
