EMPANELMENT COORDINATOR
JPS Health Network is a $950 million, tax-supported healthcare system in North Texas. Licensed for 582 beds, the network features over 25 locations across Tarrant County, with John Peter Smith Hospital a Level I Trauma Center, Tarrant County's only psychiatric emergency center, and the largest hospital-based family medical residency program in the nation. The Empanelment Coordinator is responsible for maintaining, updating, and managing the clinician-patient panels for Primary Care.
Essential Job Functions & Accountabilities :
- Ensures that each patient is assigned a Primary Care Provider.
- Empanels New Patients to providers and correct empanelment errors.
- Reviews documentation and makes decisions as to which provider should be assigned a particular patient.
- Reassigns patients as necessary based on algorithm or supervisor recommendation.
- Communicates with patients to ensure they are assigned to the appropriate clinician.
- Manages provider panel size and patient continuity (e.g. re-empanel changes to ensure appropriate access to care).
- Makes template change recommendations based on changes in clinician panel sizes.
- Instructs, and coaches new empanelment coordinators to job duties and functions.
- Maintains strict patient confidentiality in accordance with HIPAA policy.
- Monitors and reports on current empanelment status during regularly scheduled meetings with providers and the care team.

Required Qualifications :
High School Diploma, GED, or equivalent.Previous experience with Electronic Health Records.Preferred Qualifications :
