Responsibilities
As a Nurse Practitioner, you will perform annual risk assessments in a residential setting or via telehealth, adhering to your scope of practice under the guidance of a Collaborative Physician.
Essential Duties and Responsibilities
Conduct comprehensive in-home health risk assessments, identifying active and chronic health issues as well as physical, mental, and social needs during visits.Take patient histories, conduct examinations, and determine diagnoses.Document patient visits according to NCQA standards.Measure and record patient vital signs as necessary.Make case management referrals and communicate with primary care providers when necessary.Engage with patients, caregivers, agency nurses, and other providers to ensure accurate diagnoses.Follow OSHA Universal Precautions during all clinical duties.Uphold patient confidentiality at all times.Attend mandatory meetings and professional development sessions, contributing to committees as needed.You Will Collaborate With :

Required Knowledge, Skills, and Experience
Active and unrestricted Nurse Practitioner license in applicable states.Board certification from ANCC, AANP, or NCCPA.Current CPR certification.Active Medicare / Medicaid enrollment.Valid driver's license and clean driving record.Proficient in electronic health records (EHR).Preferred Knowledge, Skills, and Experience
Experience or training in geriatrics.Ability to work effectively in teams and build strong relationships with patients and staff.Conditions to Be Aware Of :
Ability to adapt to various weather and home environments.Full range of motion for lifting and handling patients, strong manual dexterity, normal vision, and hearing.Regularly lifting and carrying items weighing up to 50 pounds.Ability to travel up to 150 miles daily in urban and / or rural areas, with driving when necessary.Posted Min Pay Rate :
