What We’re Looking For :

We are currently seeking dedicated Licensed Practical Nurses (LPNs) to join our team as Care Navigators. In this full-time position, you will support patients enrolled in chronic care management and remote patient monitoring programs. Collaboration with a multidisciplinary care team, including primary and specialty practices, is key to your success.

Your Role :

• Patient Support : Conduct compassionate phone consultations with patients, providing crucial education and support for managing chronic conditions.
• Education and Empowerment : Equip patients with the knowledge and tools needed to manage their health effectively, guiding them in setting and achieving personal health goals.
• Documentation : Keep meticulous and up-to-date patient records in compliance with our protocols.
• Problem Solving : Address patient concerns, identifying practical solutions to enhance adherence and health outcomes.
• Communication : Foster clear and effective communication, ensuring patient needs are conveyed to their healthcare providers.
• Continuous Improvement : Engage in training and quality improvement initiatives aimed at enhancing patient care and navigation processes.
• Monitoring : Respond to remotely transmitted health data (such as blood pressure and glucose levels) as per approved workflows.
