Job descriptionReturn phone calls as identified by the scheduler on duty from residents, families, and external healthcare providers. Conduct thorough patient assessments to determine the urgency and nature of their clinical issues. Provide triage, clinical direction and intervention within the scope of practice. Collaborate and coordinate necessary interventions, based on the established plan of care such as contacting physicians, and communicating with healthcare providers and families / POA. Document all patient interactions accurately and completely in the electronic medical record. Collaborate effectively with other members of the healthcare team. Maintain a calm and professional demeanor in high-pressure situations. Adhere to all relevant policies, procedures, and regulatory guidelines. Registered Nurse (RN) with an active Pennsylvania nursing license. Strong assessment, critical thinking, and decision-making skills. Excellent communication and interpersonal skills.
